  • WHAT TO DO IF YOU ARE IN AN ACCIDENT…
    This is a document to print and keep in your car. It’ll guide you with what needs to be done and provide a form that will allow you to capture relevant information. You can download a PDF or a DOC. (Just click on “DOC” or “PDF“)
  • THE BACK OF THE DOOR LIST…
    This is a document to print and keep on the back of the bedroom door of each person in your home (or as photos on your phone). It’s a form that you fill in with the medical info of your loved one – allergies, medication, key medical info and medical aid details. It makes it easier to fill in forms at the hospital – especially if the person is unable to speak.  You can download a PDF or a DOC. (Just click on “DOC” or “PDF“)
  • THE PERSONAL INFO TICKLIST…
    This is a document that gives you a guideline for the kind of information that your loved ones will need if you pass away. It has been my experience that very often families experience a great deal of stress when a loved one passes away because they can’t access this information. For example, who knows the PIN for your cell-phone? Where is your will kept? This ticklist will help you plan how this information should be stored. You can download a PDF or a XLS.
